The Social Service for targeted assistance. The social project is coordinated by social workers and takes place through social secretariat interviews and social case management. In the social secretariat interviews, social workers meet beneficiaries in a welcoming space where active listening takes place, gathering the person’s history and needs. The individual is assisted in finding solutions to their difficulties and in navigating the network of services offered by OSF and local public and private organizations. Social workers facilitate the connection and case management with local services, coordinate relationships with other professionals and internal and external services to OSF for shared planning, promoting network activation and collaboration. For more complex situations, social workers assess case management within the Social Project. Case management requires medium-to-long-term intervention, where, after analyzing the situation, projects are created and implemented based on the uniqueness of the person, aimed at achieving one or more goals. The main objective is to promote the change of their situation and their well-being. To achieve the shared goals, during the case management process, intervention actions are defined based on the principles of empowerment and self-determination, always respecting the dignity of the person. A key element in the helping process is network collaboration, to encourage dialogue and relationships among professionals from various sectors, agencies, or institutions, both public and private, in order to create the conditions for synergies and shared support projects with the active participants of the project. The network with professionals includes, in addition to colleagues within OSF, social workers from other organizations, general practitioners, healthcare professionals working in hospitals, school representatives, CAF representatives, lawyers, ADS, psychologists, psychiatrists, caregivers (OSS), law enforcement, etc. Where to find us The Social Service receives visitors by appointment only at the front office of the Reception Service.
